Consent Waiver
TruNorth Pet Styling employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects from the grooming process, including but not limited to: clipper burn, exposure of the quick, small abrasions from shaving a matted coat, lethargy or soreness from standing (primarily in senior and obese pets), temporary continued shedding, and any exacerbation of a pre existing condition that is out of our control.
If a dog is exceedingly anxious or aggressive, TruNorth Pet Styling will complete the requested services to the best of their ability, but are under no obligation to continue if there is a question of safety for either pet or groomer.
If fleas are present on the pet, a flea bath will be required at that point, and you (the pet owner) agree to pay the additional fee (based on size of the dog starting at $30). You will be notified as soon as the fleas are found and a new price quote will be given at that time. If you choose to use topical flea and tick prevention it should be applied no less than 48 hours prior to the appointment.
The Rabies vaccination is a requirement for all pets in the state of Minnesota. Your dog must be current and by signing this consent form you verify they are up to date. The Bordatella vaccine is not required, but is strongly advised as the pets will share a space together.
By signing this agreement, you (the client) agree to relieve TruNorth Pet Styling LLC and it’s agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to presenting after services rendered by TruNorth Pet Styling. The pursuit of any veterinary, behavioral, or rehabilitation services, and the payment thereof, is the sole responsibility of the pet owner (unless otherwise agreed upon by the owner of TruNorth Pet Styling). Additionally, you agree to relieve TruNorth Pet Styling and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by TruNorth Pet Styling and its agents.
Because we are a by appointment only salon, we do as your dog is picked up in a timely manner (one hour after completion of service). Your stylist will confirm contact information and give you a time estimation for completion of service. Please note you may be subject to a late pick up fee ($35/hr) if you do not pick up within an hour of your completed service. We do understand things come up, but please communicate with your stylist and accommodations can be made.
